Although all newborns benefit from some level of follow-up care, post discharge monitoring is particularly critical for infants with conditions such as preterm birth, congenital heart defects, neonatal jaundice, and neonatal sepsis. These conditions predispose infants to a higher risk of post-discharge complications.
